A 49 year-old operator technician with 30 years mining experience was killed at an iron ore operation. At the time of the accident, the man was using a 20-ton rough-terrain crane to lift a section of pipeline that had been removed prior to a routine blasting operation. The outriggers were not used, and the crane tipped onto its side. According to MSHA, the victim jumped or fell as the crane was tipping over and was crushed when the crane landed on him.
