Des Moines, Neb., October 2012—A utility miner with 19 years of experience was killed on the surface of an underground limestone mine. He was driving a forklift downhill toward the mine entrance when the forklift went out of control. It struck a concrete support for a conveyor and overturned, killing him. The federal Mine Health and Safety Administration (MSHA) is investigating. MSHA reports that this was the sixth haulage-related fatality of 2012, compared to two during the same period in 2011. – www.msha.gov
